Liver xenotransplantation has recently achieved great progress. On March 10, 2024, a team from Xijing Hospital, China led the transplantation of a 50-year-old brain-dead male patient who became the first person to receive a liver transplant from a genetically engineered miniature pig and was alive for 10 days. The successful completion of the world’s first gene-edited pig-to-human liver xenotransplantation indicates a significant milestone in the field of medicine.

Although there are recent advances in the pig-to-human liver xenotransplantation, major issues associated with the procedure include immune rejection, coagulation dysregulation, cross-infection, and ethical controversy. As conveners, we would like to invite investigators in the fields of liver xenotransplantation, liver transplantation, genetic modification, xenograft rejection, and immunology to contribute review articles, original articles, editorials, or commentaries to this special issue. We are particularly interested in articles that will provide important information on whether liver xenotransplantation can keep people alive and stimulate the ongoing efforts for clinical liver xenotransplantation.
